Please note ... at present we are unable to accept appointments being cancelled via email.  We do ask that you please telephone the surgery to make or cancel an appointment.  

If you wish to request an appointment by email then please fill in as much detail as possible about the day / time of day you would prefer as well as a contact telephone number. We will then contact you with a confirmed appointment time as it may be that your requested time is not available.

We will ALWAYS contact you to confirm an appointment date and time, please do not presume that by sending an email will guarantee you that time slot.
